With our medical plans you can access care in-network or out-of-network. We recommend accessing in-network care whenever possible, as it will be less expensive. The medical plans are administered by Meritain through the Aetna Network. Your prescription drug plan is administered by SmithRx and is included for employees enrolled in a medical plan.

How do I set up or change my beneficiary?Your 401(k) account is a financial account and assigning a beneficiary is very important. It is a good idea to designate at least one beneficiary, especially if you are unmarried. If you do not have a beneficiary and you're married, your account will go to your spouse. If you are unmarried or do not have a surviving spouse, the plan document has ordering rules that govern the payment of your account. Beneficiary designation can have important tax and legal ramifications. You may want to consult with your personal financial, tax, or legal advisor before proceeding. You will need to enter you beneficiary information online at www.pcsretirement.com/login If you need help please call the help desk # at (888)621-5491. Some key definitions you will need to know are: Beneficiary type: For the first beneficiary, select Primary. Enter the remaining beneficiary information. If you want to have multiple beneficiaries, click Add Additional Beneficiary to add another beneficiary section. Beneficiary type: For additional beneficiaries, select Primary or Contingent. A contingent beneficiary will only receive a benefit if all primary beneficiaries are deceased.

Will I receive statements?Yes! You will receive quarterly statements electronically. These are available online for you to access as well as by email. It is very important that you enter your email address and remember to keep it current. This is the primary way in which information will be provided to you.

How can I get help investing?While you do direct how you want your account invested, you have the help of professional investment advisors, Granite Group Advisors – call and discuss your questions- at no additional charge to you. For those investors who would rather have someone else manage their portfolios, Granite Group offers 5 Managed Allocation Portfolios. When you invest in a Managed Portfolio, Granite Group will oversee the allocation at no additional fee to you! If you prefer to manage your own portfolio, simply select your investment choices from the vetted list of mutual funds offered through your plan. The investment menu was compiled by Granite Group Advisors and seeks to offer representation across major asset classes as well as index funds. Rest assured; this menu is actively monitored by Granite Group to ensure all options meet Granite’s strict methodology. Granite Group offers an Investment Comfort Questionnaire found at their education site www.ggahelp.com. Use the company ID of TER1 to access. Once on the site, take the Investment Comfort Questionnaire, get your score. See what Portfolio matches your score then log into you account at PCS Retirement to input your selection. Can I move other 401(k) balances from my prior employer plans into my TerrAscend Plan account?Yes! The Plan accepts rollovers from another qualified retirement plan or an IRA. There are 2 steps to consolidating your prior 401(k) plan balances into your TerrAscend account. Log into your TerrAscend account at www.pcsretirement.com/login. Under the MANAGE tab, select ROLLOVER CONTRIBUTION and you will be taken to the online form. Enter the information requested. You will be given instructions of how to send a check or a wire transfer into your account. You will need these instructions to give to the financial institution where you old 401(k) is deposited. Log into your old employer 401(k) plan and request a rollover distribution. They will ask you for the check / wire instructions provided above. What is the difference between ROTH (after-tax) and Traditional(before-tax) contributions?Contributions to your account can only be made directly from your wages. There are 2 ways to contribute and they both have to do with when you pay taxes. If you contribute before-tax (known as traditional), then you will not have to pay income taxes on the amount you contribute now. You money grows tax deferred, but when you withdrawal your money at retirement age, you will pay income tax at that future rate on all money withdrawn. Consider the following examples: If you earn $30,000 a year, are in the 22% tax bracket, are eligible to participate in the Plan, and you elect to save 5% (or $1,500) of your salary under the 401(k) Plan this year, you would save $330 in Federal income taxes (22% of $1,500 = $330). If you earn $30,000 a year, are in the 22% tax bracket, are eligible to participate in the Plan and you elect to save 8% (or $2,400) of your salary under the 401(k) Plan this year, you would save $528 in Federal income taxes (22% of $2,400 = $528). The other method of contributing is called ROTH or after-tax. In this scenario, you pay taxes now on the amount you contribute. While you do NOT get a tax advantage now, your money grows tax -free and you do not pay Federal taxes when you withdraw from your account. Your accountant can help you determine which method is the best for you.

Can I take a loan from my account?The plan does allow you to take a loan out from your account, but limits do apply. You can request a loan online or call the help desk for assistance at (888) 621-5491. To request a loan online, select the WITHDRAWALS option on the main menu of your account dashboard. Under the WITHDRAWALS menu, click onto LOANS AND WITHDRAWALS.

When am I eligible to participate?When am I eligible to participate? You are eligible to join the 401(k) Savings Plan if you are at least 18+ yrs old with 500 hours in 6-month period. You need to satisfy both requirements. If you do not meet these criteria, you will be eligible with a full year of service (1,000 hours in a 12-month period). Plan Entry date is the first day of the month coinciding with or directly following the date you satisfy the eligibility conditions. How much can I contribute?In 2024 you can contribute up to $23,000 to your account; if you are 50 years old or older, you can contribute a total of $30,500 through a catch-up contribution. Consistency is key – you take advantage of dollar cost averaging into the market – so let the “down” market days work for you!

How can I change my investments?You can change your investment elections at any time. Note that all trades are executed after markets close. When changing your investment selection, remember you have 2 places to make that change if desired. You can change how your future contributions are invested and you can also change how your existing balance is invested – this will be 2 steps online. How do I know which program to use?If your doctor determines that you need to receive surgery or imaging, before looking for an in-network provider and scheduling the procedure yourself, contact KISx Card first. The KISx Card nurse you speak with will be able to let you know if your procedure is covered. If it is covered, they will then walk you through the process of scheduling and completing the procedure. The KISx (pronounced "Kiss") program allows you to pay $0 out of pocket for over 400 surgical and imaging procedures. This benefit is available to you and your dependents if they are covered on our health insurance plan. So rather than paying your normal copay or deductible on a knee replacement, for example, you could now receive it for free! Click here to get a copy of the KISx card. Hang onto it! How do I fill a specialty prescription?A specialty prescription is for special medicines that may cost more and need extra care, often used for serious health conditions. Follow these required steps to fill a specialty prescription: Initiate the Prescription: Request your doctor to send your prescription to one of the SmithRx specialty pharmacies. Choose between Costco Specialty Pharmacy or Senderra Rx. Click here for more information. What is a Qualifying Life Event?A Qualifying Life Event (QLE) is a specific life change that allows you to make changes to your health insurance and other benefits outside of the annual Open Enrollment period. These changes are permitted because the event significantly impacts your coverage needs. Common QLEs include: Marriage, divorce, or legal separation Birth or adoption of a child Death of a dependent Loss or gain of other coverage (e.g., through a spouse's job) A significant change in employment status (e.g., starting or leaving a job, changing from full-time to part-time, or retiring) A change in residence that affects your coverage area (e.g., moving to a different state) When a QLE occurs, you have a limited window of 30 days from the date of the event to make adjustments to your benefits. This could include enrolling in, changing, or canceling coverage. It's essential to act quickly and provide any necessary documentation to avoid missing this opportunity. Where should I go for medical care?Take a look at this flyer to learn about your choices when it comes to where you should receive care. High level overview: Primary Care Physician - This is your home base. Your doctor knows your history and is a good place to start. Cost to you: Plan A - $15 copay per visit. Plan B - $20 copay per visit. Emergency Room - Go to an ER when you have a life-threatening issue. Cost to you: Plan A - $300 copay per visit. Plan B - Once you pay your deductible, there is a $300 copay. Urgent Care - Consider an urgent care center for non-life threatening condition. You'll save time and money over the ER. Why did I get billed for my preventive care visit?While regular medical care focuses on treating illness, preventive care aims to keep you from getting sick in the first place by focusing on helping you maintain good health. When you visit your doctor for a preventive care service - like a physical exam or a health screening - you should not be billed. However, if you ask your doctor about a specific health concern while you are at your preventive care visit, it may be billed as an office visit, and not a preventive care visit. Here’s how to be sure your preventive care visit is free for you: Mention prevention upfront – When scheduling your appointment, confirm that you are scheduling a preventive care visit. Know what to discuss with your doctor during the visit – During your visit, keep the conversation focused on how you can maintain a healthy lifestyle. Ask your doctor - If you do ask questions about a specific health concern while at your visit, ask your doctor if that will be treated as an office visit or as a preventive visit. What is a Coordination of Benefits and how often do I need to complete it?You are required to update your Meritain Coordination of Benefits information every year so that they can properly process your claims. Coordination of Benefits information confirms whether you and/or your enrolled dependents have insurance coverage other than your employer's and if so, what that other insurance is, so that they can determine proper payment responsibilities for claims. If your Coordination of Benefits is updated yearly or as requested, your claims will be processed. Click here for the Coordination of Benefits form. If you’d like to complete the Coordination of Benefits online, you can do so once you are registered on the Meritain portal.

How do I get a copy of my ID cards?Medical: Login to www.meritain.com. Once logged in, you'll have access to print or download an ID card. You may also call Meritain at 800-566-9311 to request a new ID card. Dental: Login at www.deltadentalins.com. Delta Dental does not mail an ID card when you enroll, but you may print out a copy on their website if you'd like. When you access dental care, you can simply let your provider know your social security number and your group number, which is 22008. Vision: VSP does not send an ID card. Instead, when you see a vision provider, simply tell them you are a VSP member and the doctor does the rest! If you do prefer to have an ID card, you can print one from the member portal after open enrollment at www.vsp.com/create-account. Once logged in, you'll be able to download a copy of your ID Card.

How do I request a Family Medical Leave or a Short-Term Disability claim?Follow these simple steps Notify your supervisor or manager of your absence from work. Using the Information Checklist, gather information about your absence. Have this information ready when you call United Healthcare. If someone makes the call for you, he or she will need to provide this information on your behalf. Call United Healthcare toll free at 1-866-556-8298. Hours of operation are Monday through Friday, 8 a.m. – 6 p.m. ET. If your absence from work is due to your own health condition, please sign and date this Authorization Form. Give your physician the signed and dated form. Please also fax a copy of the signed, dated form to United Healthcare at 1-866-334-0985.

Does TerrAscend provide Voluntary Life Insurance?TerrAscend offers voluntary life insurance and AD&D through United Healthcare. This voluntary coverage will enhance the limit provided to you by TerrAscend. You may choose coverage at a minimum of $10,000 and a maximum of $500,000, with a guaranteed issue limit of $150,000 for employees, $25,000 for spouses, and $10,000 for children. Coverage is offered in increments of $5,000 for employee and spouse, and $1,000 for children. Coverage for you as an employee may not exceed 5x your salary. Coverage for your spouse cannot exceed 50% of your election and coverage for your child(ren) cannot exceed 10% of your election. There are certain instances when you may need to complete an Evidence of Insurability form. For example, if you would like to elect coverage greater than the guaranteed issue amount, or if you would like to elect increments greater than listed above, please complete the Evidence of Insurability form. Follow directions on the form to complete and submit the form to United Healthcare. Please contact your Viridis benefits advocate for any questions.
